Welcome to the FACT Inspector Application Process!

One of the greatest benefits of serving as an inspector is the opportunity to give back to the field of cellular therapy and help organizations provide patients with the best possible care for patients. FACT is committed to serving its inspectors and providing resources for conducting inspections and improving their own organizations’ practices. If you're ready to take the next step, begin the application process by gathering the documentation outlined below and completing the steps (on the right), beginning with Registration and Acknowledgement.

Please have the following documents on your computer ready to be uploaded with this application. 

  1. Current Photo (head shot) requested within the inspector application (with file extension of .jpg or .png)

  2. Medical License & CV/Resume (in one document with one of the accepted file extensions: .doc, .docx, .xls, .xlsx, .ppt, .pptx, .pdf, .gif, .jpg, .jpeg, .png)

Inspector Application Registration and Acknowledgement
Registration is for individuals who wish to apply to become a FACT Inspector. You must register to access.
Initial FACT Inspector Qualifications
Information on the qualification requirements to become a FACT inspector. You must register to access.
Complete Inspector Application
CLICK on the COMPLETE APPLICATION button. The application will open. Once complete, continue to upload the required documentation. You will receive an "Approved," "Resubmission Required," or "Not Approved" email from FACT. If approved, you will be provided further instructions on the next steps. If denied, you will receive a reason for denial. If a resubmission is required, you will receive instructions for correction via email. You must register to access.

Submit Medical License & CV/Resume
Gather the following documents and information to upload in this module: 1. For inspectors who are physicians (MD, DO, MBBS), medical license verification or electronic copy of their current medical license. 2. Current curriculum vitae or resume in electronic format. Submit both as one document using the submit button. You must register to access.
